Job Title: Behavioral Health Care Coordinator
Location: West Ft. Worth, TX – Areas Covered: Western Hills, Benbrook, Wedgewood, and Westover Hills
Key Responsibilities:
- Conduct comprehensive assessments of referred members' needs and eligibility to determine the best approach to case resolution.
- Complete face-to-face visits and monthly phone contact with members, utilizing various treatment modalities and interventions to achieve desired outcomes.
- Apply critical thinking and judgment to support and guide the service coordination process, facilitating appropriate healthcare outcomes.
- Educate and empower members to ensure compliance, satisfaction, and promote patient advocacy.
- Identify opportunities for quality improvement and follow through with corrective actions when necessary.
- Promote and engage members in behavior changes that will lead to better health outcomes using motivational interviewing techniques.
- Facilitate communication and collaboration with team members and other healthcare providers.
Position Requirements:
- Experience: 2 years of field case management experience preferred.
- Education: Bachelor’s Degree (preferred fields: psychology, social work, counseling, sociology, human growth and development, or early childhood education). Non-Licensed Master-Level Clinicians are also welcome to apply.
- Language: Bilingual (Spanish) is preferred but not required.
- Travel: Ability to travel up to 75% of the time within the service delivery area. Mileage reimbursement is provided.
