Description
Back-up Staff needed for AFL Provider to provide services in your home for Individual when the AFL Provider is in need of back up support.
PURPOSE: To provide an inclusive family environment in which an individual with developmental disabilities can have a meaningful and fulfilled home life.
POLICY: ComServ, Inc. provides residential care (24-hour services) for individuals who have developmental disability/MH/SA in YOUR home environment. The primary purpose of these services is to provide personal care and habilitation in a residential setting.
The services are provided in the primary residence of a family and can be licensed or unlicensed.
The Division of Health Services Regulations will license the home when the family serves either one or more minors or two or more adults who require residential services. The license will be completed under ComServ, Inc. and the agency will have sole responsibility for placement of individuals in the home. AFL families are a form of natural supports identified as places, things and, particularly, people who are a part of individual’s interdependent lives and whose relationships are reciprocal in nature and often vital to an individual’s welfare.
The concept is that purchased or funded supports should not supplant natural resources available to the individual when they are available and appropriate. With this in mind, the following guidelines are provided for waiver services furnished by family members or AFL families.
AFL providers are required to complete the ComServ, Inc. AFL provider application.
One or both family members will be contracted for services provided in their residence through ComServ, Inc. The family member on contract will be responsible for submitting a monthly invoice for services provided to the Qualified Professional who supervises the services or other designated person.
AFL Providers are not eligible for benefits through ComServ, Inc., but will be issued a 1099 form at the end of the year.
AFL Providers will be responsible for completing all of the required training for regular employment through ComServ, Inc.
All individuals in the home over the age of sixteen will be required to complete a criminal background check and a drug screening.
Individuals served in the home can only be left with an adult who has completed ComServ, Inc. Training.
Any person found to have a criminal background cannot be allowed to be alone with an individual served, even with guardian permission.
Contracted AFL Providers will follow all procedures of ComServ, Inc. Policy and Procedure Manual in Section I and any other applicable policy.
AFL providers will follow the same preservice delivery process as all ComServ, Inc. employees after completing the AFL Provider application. A competency file will be maintained on each AFL provider in addition to the home file.
