Job Description
The Consulting Dietitian provides overall nutrition care to individuals on an as-needed basis.
40 hours per month is per contract.
Ensures nutritional needs of individual patients are being met and all documentation, as it relates to the individual’s nutritional status, is complete and accurate. Plans and administers activities related to individuals nutritional care, complying with the standards established by regulatory agencies, and clients. Establishes and maintains effective multidisciplinary relationships to provide a unified approach to individual/resident care.
Job Responsibilities
- Conducts patient nutrition assessment on assigned individual population
- Manages the development and implementation plan for nutrition intervention in accordance with the Nutrition Care Process
- meets with individuals face to face on-site
- Provides nutrition counseling for individuals and groups, taking into consideration any adaptations to teaching methods necessary to meet individuals learning needs
- Utilizes assessment and evaluation techniques that consider the varied needs of age-specific populations as well as cultural, religious, and ethnic concerns
- provides recommendations and prescribes diets (modified and special)
- Evaluates achievement for quarterly drug reviews,
- Continually monitor operations, and complete assessments and necessary action plans to provide optimal nutrition care thru annual evaluations.
- Works cooperatively to train staff as well as nutrition services staff to assure conformance to prescribed nutrition diets.
- Provides education to staff that ensures compliance of dietary needs.
- Provides appropriate and timely documentation that summarizes the nutrition care plan in the individuals medical record, including nutrition assessment, diagnosis, plan, implementation, and progress towards goals
- Demonstrates accountability for the proper use of individuals protected health information
Qualifications
Must be registered by the American Dietetic Association
Bachelor's Degree in foods and nutrition, dietetics, or related area from an appropriately accredited institution that is accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) of the Academy of Nutrition and Dietetics (AND) and current registration in good standing, administered by the Commission on Dietetic Registration (CDR)
two years of experience in a clinical dietary setting;
or an equivalent combination of education and experience.
Earn and maintain applicable certifications, registrations, licenses, credentials and continuing education requirements by the appropriate accredited local agencies
Formal eligibility to practice dietetics in geographic location where applicable
