Description
Comuniilfe Inc. is looking for an experienced Case Manager for its transitional residential Program in the Bronx, Safe Haven I. The Case Manager’s main responsibility is to provide comprehensive case management services to assigned clients and their family members. The Case Manager is responsible for full coordination of services for all clients assigned; ensure that all assessments and service plans are completed and all required/needed services are rendered or arranged based on service plans' specifications. Responsible to conduct room visits and inspections. Qualifications
Education and Experience Requirement(s)
Bachelors Degree required; with minimum two years field work experience and direct mental illness work experience. Individual, family and group counseling experience required, particularly with mentally ill populations. Preference given to persons with experience in working with homeless individuals in Congregate Programs, bi-lingual, bi-cultural in Spanish, a plus.
