We are currently seeking a highly skilled Case Manager to join our team in Port Jefferson, New York. The successful candidate will have a Bachelor's Degree in Psychology, Rehabilitation, Social Work, or a related mental health discipline, plus 2 years of experience working in a similar setting. They should possess excellent verbal and written communication skills, as well as the ability to make sound judgments regarding client care.
