Summary/Objective
Concerto is seeking an EQRS Specialist with demonstrated experience in cross departmental collaboration and effective communication. Our model of dialysis care is governed by multi-state Networks utilizing various platforms requiring timely submission of patient demographics, admissions, discharges, and applicable filing of legal documentation as mandated by the Centers for Medicare and Medicaid Services (CMS). This person will act as a liaison between facility staff to include nurses and social workers as well as collaboration with the admissions team, billing team, and market leaders. With the support of the Director of Social Services and our Senior Crown Web Specialist, this individual will be responsible for identifying and resolving and/or escalating system issues to ensure successful outcomes. This position reports to the Director of Quality. This role is hybrid with three (3) days on-site in Lincolnwood, IL and the remaining two (2) days remote.
Essential Functions
- Navigate complex data collection systems across multiple markets (states).
- Ability to work independently with little oversight yet have on demand support for training, troubleshooting, and job fulfillment needs.
- Actively completes all manual entries and/or group submissions including collecting, organizing, and confirming accuracy of patient specific data before it is submitted.
- Identify, research, and correct system issues that could prevent timely submission and achievement of 100% data entry each month.
- Manage insurance related documents and submit to the Social Security Administration office.
- Provide a high level of customer service to field staff, ensuring all questions and issues are effectively answered and addressed appropriately.
- Identify and implement process improvement for current and future data entry processes.
- Attend team meetings via in-person, phone, or Microsoft Teams bi-weekly.
- Work cross functionally with social workers, clinical disciplines, leadership teams, the billing, and the admissions department.
Qualifications
- Proficient in MS Excel and Word
- 1-3 years of Customer Service and data entry experience.
- EQRS (formerly Crown Web) experience required.
- Excellent time management skills and ability to prioritize work independently.
- Strong attention to detail and data tracking methods.
- High School diploma or equivalent
Compensation

$20 - $24 per hour

Hourly rate is dependent upon several factors, including but not limited to the following: a candidate’s experience in this position and/or the dialysis industry in general, location of the position in question, urgency of company need, timeline of required to fill position, and other potential and/or applicable factors that emerge from time to time.
