The ConcertoCare Assistant Clinical Director manages clinical care delivery and performance in performance for a team of Advanced Practice Providers in collaboration with the Medical Director. Key activities include direct patient care, support of clinical care delivered by field-based clinical practitioners, data-driven and KPI-directed clinical performance management, relationship support for key partnerships, and daily personnel management for clinical practitioners. This position reports to the Medical Director.

This is an ideal position for an Advanced Practice Provider who seeks an opportunity for "top of license" clinical practice and leadership supporting a team of advanced practice providers, enjoys a collaborative multidisciplinary team-based approach to care, and is excited to engage in developing and nurturing our innovative, value-based clinical model focused on focused on caring for patients with complex needs who are inadequately served by traditional healthcare delivery systems.

RESPONSIBILITIES

  • Direct patient care including annual wellness, continuity care, urgent care, and care transitions visit types in home, clinic, congregate living, and / or skilled nursing facility-based settings.
  • Clinical support for care delivered by clinical provider team, including but not limited to support of care quality, clinical decision making, and compliance.
  • Management of local provider teams including hiring, training, coaching, counseling, supporting, and evaluating direct reports.
  • Onboarding support and clinical training for new clinical team members.
  • Participation in care team huddles to provide clinical input, support individualized care plan development, and champion value-based care strategies to drive clinical outcomes and performance.
  • Clinical performance management including ongoing clinical data evaluation and strategic deployment of clinical team resources to achieve KPI targets.
  • Collaborative partnership with market leaders including engagement in strategic design, planning, and implementation of data-driven initiatives, processes, and programs to meet performance targets.
  • Clinical liaison for local clinical community partnerships including providers, SNF, hospital, and other clinical partners to support individual patient care needs and to ensure successful ongoing clinical collaboration, communication, and performance improvement.
  • Engagement in developing and implementing new clinical initiatives and championing change with the local team for national operational workflows, technology innovation, and care quality initiatives.
  • Adherence to medical policies, protocols, criteria, and clinical guidelines to ensure best practices are maintained for clinically effective and efficient care delivery.
  • Adherence to compliance policies, procedures, and standards of conduct including all applicable laws and regulations including promotion of compliant communication and behaviors amongst all team members.
  • Participation in the after-hours on-call schedule as required.
  • Other organizational and / or market-specific duties as assigned.

QUALIFICATIONS

  • Nurse Practitioner or Physician Assistant with graduation from an accredited health professions school and active national board certification.
  • Active licensure in applicable state(s).
  • Active DEA licensure and state-specific controlled substance registration, as applicable.
  • Certification in basic life support (BLS) required.
  • Clinical experience caring for aging adults and adults with complex care needs required.
  • 3 years clinical practice experience as a provider preferred.
  • Advanced training in geriatrics and / or palliative medicine preferred.
  • Clinical experience caring for patients in the home setting including private homes and congregate living facilities preferred.
  • Clinical team leadership experience preferred.
  • Public health, business administration, and / or health administration studies preferred.
  • Previous experience with managed care and / or value-based healthcare delivery preferred.
  • Average to advanced computer and software skills required.
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS : Licensed for a minimum of 3 5 years. No vehicle-related suspensions / reinstatements, DUI, reckless driving, or felony convictions within the last 7 years. Multiple violations and accidents within the last 5 years would be subject to review.
  • Base Salary / Wage Range $165,000 to $175,000 plus annual bonus . Compensation for the role is commensurate with the candidate's qualifications, skills, competencies, and experience and may fall outside of the range shown.
