GENERAL DESCRIPTION: The employee provides Community Living Support (CLS) skills training practice to children and families participating in the Youth Empowerment Services (YES) Waiver. The employee works in collaboration with Waiver participants and families towards the fulfillment of unmet underlying needs specifically: community inclusion and maintain stability in the home environment. The employee will provide practice in skills related to activities of daily living and socialization. They will work to reinforce skills or lessons taught in school, therapy, or other settings. This practice may include: basic education about the nature of the mental illness, the importance of consistent self-administration of medication as prescribed, and/or basic parenting skills. The employee will assist individuals and families in gaining access to medical, social, educational and other appropriate services that will help the child achieve a quality of life and level of care acceptable to the family. This employee will also provide crisis prevention, intervention, and management services to include location and coordinating services and supports to prevent or manage a crisis. They will serve as an active member of the child family team (LOC YES) unless deemed unnecessary by the child family team itself.
ADDENDUM: The employee will provide traditional skills training to individuals within the MHMRCV catchment area who are identified as qualifying for packages which skills training is deemed applicable &/or necessary for improvement. As Case Manager I/CLS, this employee will be held to expectations of productivity as designated by the Intensive Services Supervisor and/or the MH Program Manager as well as individual child and Family teams within the wraparound process.
EDUCATION REQUIRED: Bachelor Degree in a Center approved Human Services field. Bachelor or advanced Degree from an accredited college or university with a major in a social, behavioral, or human service field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, and education.
LICENSE/CERTIFICATION: Valid Texas driver’s license. Must be insurable on the Center’s insurance. No more than 3 moving violations in the last 3 years. Ability to be credentialed as a Qualified Mental Health Professional-CS.
EXPERIENCE REQUIRED: An Undergraduate degree in any of the disciplines listed above, at least 1 or more years working with families and children, experience/knowledge of mental illness and treatments for improving mental health, knowledge of community support service agencies as well as knowledge of the community.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: SPECIAL SKILLS: Willing to work in an accountable system where goals for performance are established and measured and being held accountable for meeting or exceeding those established goals. Ability to perform tasks requiring attention to detail. Ability to work with special population. Ability to type accurately. Must be able to maintain professional boundaries with coworkers and consumers. Ability to communicate effectively with professional clinical staff. Demonstrated organizational skills. Ability to prioritize work and meet deadlines. Computer literate. Ability to multi-task in a fast-paced environment.
SHIFT: Full time: 40 hours per week.
