GENERAL DESCRIPTION:
The employee will provide recovery-oriented services as outlined by the Texas Administrative Code and in accordance with the Texas Health and Human Service Commission Resiliency and Recovery Management principles. Specifically – the employee will provide Mental Health Case Management functions as outlined by Texas Administrative Code, Title 25, Part II Service Coordination Chapter 412, Subchapter I to a designated caseload of TCOOMMI individuals that have been referred from local Adult Probation & Parole Department. Services include assessment, treatment planning, monitoring of services, skills training, case management and crisis prevention and management. Employee will develop and maintain referral resources for service provision allowing for consumer choice and comply with Texas Correctional Office on Offenders with Mental/Medical Impairments (TCOOMMI) Program Guidelines. Employee will ensure best value of resource allocation to service provision. The focus of services is to stabilize symptoms, reduce the risk of recidivism, increase awareness of and participation with community and natural supports, develop skills in self-advocacy and gain ability to participate in independent mental health care. Also, this employee will be cross trained with other TCOOMMI Programs with MHMR Services for the Concho Valley to provide TCOOMMI coverage as needed. The employee will implement the principles of trauma-informed care including culture sensitivity in all areas of service delivery.
EDUCATION/QUALIFICATIONS:
- Bachelor or advanced Degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
- One year of experience in a similar or related position, either public or private sector
EXPERIENCE AND SKILLS REQUIRED:
- Prefer (1 to 2) years’ experience in like or similar position, either public or private.
- Education will be considered in lieu of experience.
- Preference will be given to individuals that have training and experience with the Texas Recommended Authorization Guidelines Assessment protocols.
- Preference will be given for experience in the Texas Community MHMR System and knowledge of the Resiliency and Disease Management Utilization Management Guidelines.
- Strong communication skills
- Ability to work in a rapid pace environment in an organized and professional manner.
- Ability to self-monitor production and quality with minimal assistance.
- Proficient in the use of electronic data systems including electronic health records. and business software such as Microsoft Word and Excel.
- Organization and typing skills are a must.
- The ability to work as a team member in a diverse environment interfacing with both internal and external referrals.
REQUIREMENTS:
- Valid Texas driver’s license
- Must be insurable under the Center’s insurance policy
- No more than 3 moving violations in the last 3 years
- Must be able to pass a NCIC and TCIC Criminal Background Check prior to employment.
- Must be insurable and remain insurable on MHMR Services for The Concho Valley insurance.
SHIFT
Monday through Friday, 8:00 a.m. to 5:00 p.m.
