Job Overview
As a Registered Nurse, you will play a crucial role in providing high-quality patient care in a home setting. Your expertise and compassion will be instrumental in ensuring the well-being of patients under your care. The main responsibilities will be completing an intake assessment for new clients in their residence and completing periodic re-assessments. Additional duties may include delegating nursing functions in accordance with the client’s needs and all applicable regulations.
This job will primarily be in the community, with some office days required to maintain and update documentation.
Duties
- Completes the admission process appropriately for non-skilled services per regulatory standards and per the policies and procedures of Concierge Home Care of Maryland
- Performs periodic assessment of client’s functional and medical status
- Collaborate with patient support planner or physician for plan of service in the home
- Provides supervision for home health aide, personal care aide, and CNA as appropriate per the policies of Concierge Home Care of Maryland
- Deliver services with empathy and professionalism
Qualifications
- Valid RN license in the state of Maryland
- CPR/First Aid Certification
- 2-3 years experience as a Registered Nurse
-Prior experience in home care preferred, positive attitude a must!
- Experience with EHR- electronic health record systems
- Experience with Medicaid in home care programs preferred
- Ability to provide care with compassion
- Excellent communication and interpersonal skills
