Clinical Social Worker (CSW) Job Duties and Responsibilities:
Evaluating clients to determine their needs and the support options that can help them
Creating individualized care plans for each client
Referring clients to community resources to help their specific situations
Collaborating with other care providers to coordinate services
Monitoring clients
Maintaining records of treatments and services
Providing talk therapy
Responding to crisis situations
Additional job duties and responsibilities as assigned by job description, administrator, Director of Nursing (DON), and Regional support team.
- Licensed Clinical Social Worker (LCSW) is preferred but not required.
