Education
PharmD degree or Bachelor’s degree in Pharmacy from an accredited program.
Certification, Registration & Licensure
Licensure required: NH Pharmacist license or reciprocity pending.
Board Certified Pharmacotherapy Specialist (BCPS) is desirable.
Experience
A Pharmacy residency is desirable.

Responsibilities
- Identifies and resolves actual or potential medication related problems.
- Takes ownership of pharmacy related issues and follows them to completion.
- Provides accurate drug information to staff, providers, and the community.
- Assist physicians and residents to develop optimal drug therapies for each patient, including
- Establish a pharmacy practice specialty or an area of expertise and act as a resource for this specialty,
department.
- Documents and monitors all interventions and their outcomes.
- Performs medication order entry in a manner consistent with current department policies and procedures.
- Coordinates the daily distribution process of the pharmacy.
- Represents the pharmacy in hospital-wide committees. Participates in pharmacy committees.
- Provides written and/or in- service education on a regular basis.
- Performs other projects or duties as required or assigned.
