What are the responsibilities and job description for the Palliative Care Program Coordinator position at Concord Hospital?
Summary
As a member of the Palliative Care Consult team, provides multidimensional assessments and clinical interventions in a counseling capacity to assist individuals and families to restore, enhance, or protect their capacity for psychosocial functioning and personal meaning, in coping with the consequences of a chronic disease process or life threatening illness. Provides consultation and recommendations to the providers and interdisciplinary team members of the referring service to effect successful outcomes. Works closely with the Medical Director in facilitating the development, coordination and continuous quality improvement of a comprehensive Palliative Care Program.
Education
Master's level education Social Work or other relevant degree.
Certification, Registration, and Licensure
Licensure required: Valid New Hampshire Board of Mental Health Licensure (LICSW) or other relevant counseling licensure.
Experience
Minimum of 3 years experience in counseling, palliative care, social work or other relevant field.
Responsibilities
- Provides palliative care counseling to individuals, families or support system (guardian, significant other, etc.) that supports goals setting and decision making to promote optimal care planning.
- Assesses patient and family needs.
- Collaborates with, provides consultation / recommendations to, and functions as part of an interdisciplinary team in promoting appropriate care for patients and families.
- Coordinates, analyzes and enhances program care delivery and care systems.
- Maintains medical records.
