Concordia Nursing and Rehab is searching for a Director of Nursing to join our amazing team! We value teamwork and positivity in our facility and need someone to help lead that initiative. The position of Director of Nursing (DON) is an all encompassing one. A DON must possess the necessary clinical and managerial skills to direct and lead a nursing department. It is vital that the person communicate clearly to residents, their families, nurses and all nursing home personnel. The DON must lead the nursing staff in taking positive action to continually upgrade the quality of physical, social, and emotional care given. The DON must be able to serve as a charge nurse on the floor as staffing needs may require them to do so. Our facility typically has an average census between 40 and 50 residents. The DON will have a flexible schedule that can accommodate days off during the week/working weekends.
QUALIFICATIONS
o Must be a Registered Nurse in good standing and currently licensed by the state.
o No less than two (2) years previous long-term care experience and no less than one (1) year of Director of Nursing experience.
o Experience in management and geriatric nursing.

PHYSICAL REQUIREMENTS OF JOB (with or without assistance):
Walking, reaching, bending, lifting, grasping, pushing and pulling, fine hand coordination, ability to hear and respond to pages, ability to understand and follow written and oral instructions and directions; ability to communicate with residents, families, personnel, vendors and consultants; ability to distinguish smells, tastes, and temperatures; and ability to remain calm under stress.
RESPONSIBILITIES
o Plan, organize and direct the administration of all nursing units and patient care given based on established goals and objectives, standard, policies and procedures of the company and facility.
o Put into effect the administrative policies of the company. Maintain required records. Review, update and revise policies including OBRA procedures to meet current objectives and state and federal regulations.
o Regularly inspect the facility, nursing practices and documentation for compliance with federal, state and any local standards and regulations.
o Assure patients of a comfortable, clean, orderly and safe environment.
o With designee of choice, interviews all RN and LPN applicants for the nursing department and hire or reject after reviewing references. Responsible for terminating associates if their work is not satisfactory or who is in violation of company policies (after counseling and formal warnings) following company policy. All terminations are to be discussed with administrator. Oversees all RN & LPN evaluations.
o Insure that all shifts are adequately covered for nursing services following state and federal policies as well as patient needs.
o Maintain and monitor procedures for administration and control of medication and policies for the care, use and stocking of all nursing supplies and equipment.
o Oversee agenda preparation for medical staff and utilization review meetings. Function as the liaison between state and federal agencies in regards to Medicaid, Medicare and any and all other insurances. Complete any required documentation in a timely manner.
o Meet daily with critical core team members regarding admission, placement or discharge of patients. In addition, participates in coordination of patient services through departmental staff meetings and assists in the development of patient’s care plans. Oversee the complete and timely completion of care plans. Attend department head/administration meetings.
o Review all infection control reports, medication incident reports and I&A reports. With appropriate staff, develop corrective action plan. Study all weekly reports such as level of care reports, dietary and pharmacy consultant reports.
o Meet monthly with staff on each shift. Provide in-services to all shifts as necessary to maintain a quality nursing program.
o Maintain all required records. Meet monthly with the nursing staff regarding chart audits and physician orders. Perform in-house quality assurance surveys on a quarterly basis and maintains all quality assurance requirements and recommendations.
o Is on call for emergencies that other supervisory personnel cannot handle.
o Work closely with the administrator in setting and evaluating the nursing department budget. Recommend the number of level of nursing personnel to be employed within sound fiscal guidelines and quality patient care.
o Define job descriptions for all nursing personnel
o Working knowledge of company policies and procedures
o Stay up to date on state and federal regulations and policies
o Plan, organize and direct all patient care
o To perform other duties as requested by management.
