The Licensed Practical Nurse (LPN) provides skilled nursing care to hospice patients in accordance with the individualized plan of care developed by the patient and/or family, physician and hospice team. Care for the patient is given where the patient resides (i.e., patient’s home, facility, hospital).
QUALIFICATIONS
- Current LPN license in the state of Pennsylvania
- Minimum of two [2] years experience in patient care preferred
- Demonstrates excellent clinical skills
- Current CPR certification
- Solid computer skills, including Word
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
- Reliable independent means of transportation with a valid Pennsylvania driver’s license and at least the minimum state required vehicle insurance
