Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. Good Samaritan Hospice is seeking a full time Triage Nurse to work remotely to assist our patients in our service territory. The Triage Nurse is a licensed health care professional who is responsible for communicating with our patients and/or families after hours (from 5 pm to 8 am) assessing their needs in an efficient and effective manner which includes assigning a clinician to make a visit to the patient if necessary. This position would also assist with workflow duties and includes a weekend and holiday rotation since hospice is a 7 day/week serviceline.

QUALIFICATIONS
- Registered Professional nurse with current valid Pennsylvania license
- Minimum of [2] years of current experience as a professional nurse in a medical/surgical environment and [1] year home health experience preferred.
- Skilled in critical thinking, communication, negotiation, collaboration and customer service.
