130 - Nurse Aide/HHA - Certified Nursing Assistant
Concordia Hospice of Washington is hiring for a full time Home Health Aide to cover our Canonsburg and surrounding areas communities!
POSITION SUMMARY
The Hospice Aide provides personal care and assists in activities of daily living and related services to hospice patients. This employee functions under the direction, instruction and supervision of the Registered Nurse (RN). Care for the patient is given where the patient resides (i.e., patient’s home, facility, hospital).
QUALIFICATIONS
Current Certified Nurse Aide (CNA) in the state of Pennsylvania preferred
Minimum of two [2] years experience in patient care preferred
Ability to understand and carry out directions and instructions and maintain appropriate records
Current CPR or First Aid certification
Excellent organizational skills with attention to detail
Outstanding communication skills (both written and verbal)
Reliable independent means of transportation with a valid Pennsylvania driver’s license and at least the minimum state required vehicle insurance
RESPONSIBILITIES
1. Provides personal care according to the established individualized plan of care as developed by the RN member of the hospice team such as (but not limited to): bathing, grooming, dressing, shaving, feeding; checking vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation
2. Coordinates and communicates a weekly patient schedule with the RN Case Manager and Licensed Practical Nurse (LPN) to meet the optimal needs of the patient and family
3. Completes all computer and/or paper documentation requirements within the timeframes specified
4. Participates in patient and/or family teaching activities as directed by the RN hospice team member
5. Together with the hospice team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death
6. Maintains regular communication and interaction with patients, families, physicians and hospice team members
7. Participates in required on-call responsibilities
8. Attends and participates in staff meetings and other related activities when required
9. Completes required in-services; ensures all required continuing education credits are obtained for certification renewal if applicable
10. Demonstrates an understanding of the Quality Assessment Performance Improvement (QAPI) process, hospice policies and procedures, and participates in projects when requested
11. Participates in the orientation of new staff members when requested
12. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and Concordia Hospice of Washington policies and procedures; maintains confidentiality of patient and employee records and information; has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to the Executive Director and/or Compliance Manager
