POSITION SUMMARY for the Hospice Licensed Practical Nurse (LPN)The Licensed Practical Nurse (LPN) provides skilled nursing care to hospice patients in accordance with the individualized plan of care developed by the patient and/or family, physician and hospice team. Care for the patient is given where the patient resides (i.e., patient’s home, facility, hospital).
