The Clinical Supervisor – LPN is a licensed healthcare professional that is responsible for ensuring that patient care is coordinated and managed appropriately according to state and federal guidelines. The Clinical Supervisor - LPN assists the interdisciplinary team in planning, implementing, and evaluating the patient plan of care for appropriateness to the individual patient’s needs to ensure that high quality healthcare is delivered.
QUALIFICATIONS
Licensed Practical Nurse with a current, valid Pennsylvania license.
OASIS Certified preferred
Two [2] years of progressive experience in a home health care setting preferred.
Interpersonal, communication and organizational skills.
Proficient with knowledge of an electronic health record.
Knowledge of agency policies, federal and state regulations and third-party payers
Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
RESPONSIBILITIES
Reviews clinical documentation for compliance and completeness in a timely manner.
Reviews vital sign alert reports and notifies physician of negative trending
Oversees and offers supervision to members of their team to help assure that quality of care is provided.
Evaluation of patient’s condition for the need of other services, equipment, and consultations that may facilitate better overall outcomes.
Holds regular meetings to provide education to team members and facilitate interdisciplinary communication.
In collaboration with the primary clinician, provides what is necessary for the patient’s need to obtain positive medical outcomes while managing appropriate resources.
Plans with the primary clinician and other disciplines for the discharge from service at the time of admission.
Knowledge about payer processes and requirements to bridge the gap between provider and payer and to effectively meet the needs of both, ultimately for the patient’s benefit.
Liaison between patient/family, physician and all other healthcare providers.
Provides in-home nursing visits on an as-needed basis.
Participates in administrative on-call rotation.
Responsible for understanding patient outcome and HHCAHPS requirements and assisting with education of the staff as it relates to these regulatory requirements.
Reviews Telehealth data and identifies negative results. Collaborates with patient, clinical staff and physician when negative results or trending are identified.
Knowledgeable about agency requirements for Telehealth monitoring, including the appropriateness of the patient, orders and forms needed, agency- specific protocols, basic equipment functioning along with appropriate cleaning methods and infection control.
Is able to provide correct teaching on use of the home telemonitoring unit as well as basic troubleshooting to patients, designated representatives and other clinicians as needed. Familiar with the appropriate channels of communication for any problems or concerns with home telemonitoring unit.
Reviews and approves the referral for accuracy and completeness in the electronic medical record. Ensures Intake communication note is thorough and complete.
