Concordia-IRMC VNA is hiring for a Certified Registered Nurse Practitioner (CRNP)-Palliative Care. We are seeking a skilled Certified Registered Nurse Practitioner (CRNP) to join our palliative care team, where you will play a pivotal role in enhancing the quality of life for our patients. This position will be supporting our patients within our service territory and will work closely with our partner hospital Indiana Regional Medical Center (IRMC).

Key Responsibilities for the Certified Registered Nurse Practitioner (CRNP)-Palliative Care Position:
Goals of Care Conversations: Engage in thoughtful and empathetic discussions with patients and families to establish and navigate their care goals.
Comfort Medication Prescribing: Prescribe and manage comfort medications to ensure the highest level of patient comfort and quality of life.
Face-to-Face Encounters: Conduct thorough, face-to-face patient assessments to provide personalized care and support.
Case Management: Oversee and coordinate comprehensive patient care, working closely with interdisciplinary teams to ensure cohesive and effective treatment plans.
Qualifications for the Certified Registered Nurse Practitioner (CRNP) position:
Current PA Certified Registered Nurse Practitioner (CRNP) license
Palliative and/or hospice experience
Proven experience in goals of care conversations, prescribing comfort medications, case management, and performing face-to-face patient encounters.
Demonstrates excellent clinical skills
Current CPR certification
Strong communication and interpersonal skills with a compassionate approach to patient care.
Solid computer skills, including knowledge of Microsoft Office
Excellent organizational skills with attention to detail
Knowledgeable of federal and state regulations for home health and hospice
