Key Responsibilities for the Certified Registered Nurse Practitioner CRNP Care Position:
Goals of Care Conversations: Engage in thoughtful and empathetic discussions with patients and families to establish and navigate their care goals.
Comfort Medication Prescribing: Prescribe and manage comfort medications to ensure the highest level of patient comfort and quality of life.
Face-to-Face Encounters: Conduct thorough, face-to-face patient assessments to provide personalized care and support.
Case Management: Oversee and coordinate comprehensive patient care, working closely with interdisciplinary teams to ensure cohesive and effective treatment plans.
Qualifications for the Certified Registered Nurse Practitioner CRNP position:
Current PA Certified Registered Nurse Practitioner (CRNP) license
Demonstrates excellent clinical skills
Current CPR certification
Strong communication and interpersonal skills with a compassionate approach to patient care.
Solid computer skills, including knowledge of Microsoft Office
Excellent organizational skills with attention to detail
Knowledgeable of federal and state regulations for home health
