Position SummaryThe Nurse Aide assists the licensed nursing staff by providing direct resident care related to Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Nurse Aide Shift Available: Daylight: 6:00am-2:30pm, every other weekend ( 1.00 on weekends)Afternoon: 2:00pm-10:30pm, every other weekend ($1.00 shift differential, 1.00 on weekends)Qualifications:Caring, compassionate, and understandingHave a high school diploma, GED diploma or active registry status on the Pennsylvania nurse aide registry18 years of age or olderBe free from a medical condition, including drug or alcohol addiction that would limit the staff person from providing necessary personal care services with reasonable skill and safetyExcellent customer service and hospitality skillsNurse Aides with Medication Administration responsibilities will need additional qualifications: 1) successful completion of the DPW-approved medications course that includes the passing of the DPW’s performance-based competency test within the past 2 years to be able to administer oral, topical, eye, nose and ear drop prescription medication, and epinephrine injections for insect bites or other allergies2) successful completion of a DPW-approved diabetes patient education program within the past 12 months to be able to administer insulin injectionsEssential Duties and Responsibilities1. Assists with Activities of Daily Living (ADLs) consistent with resident support plan by assisting residents with eating and drinking, ambulating, transferring, toileting, bladder and bowel management, personal hygiene, bathing, dressing, turning and positioning, and documenting the care that was provided. 2. Assists with Instrumental Activities of Daily Living (IADLs) consistent with resident support plan by assisting residents with doing laundry, using a telephone, caring for personal possessions, changing linens, obtaining and keeping clean, seasonal clothing, using a prosthetic device, keeping a safe, neat, clean room, and transporting to activities.3. Completes over-the-counter treatments per printed instructions in the ADL/treatment book by reading what treatment is ordered for the resident, providing the correct treatment in a timely manner, and documenting that the treatment was done (paper chart and EMR).4. Answers resident call bells for assistance by responding to the call bell in 4 minutes or less, knocking before entering resident room, prioritizing resident needs, addressing resident need, and turning call bell off.5. Assists residents in accessing health care by communicating important resident observations, changes in condition, findings, and problems to the Charge Nurse in a timely and proactive manner.6. Commits to high ethical standards by complying with the Code of Conduct, state and federal regulations, accreditation standards, Concordia policies, procedures, and safety standards, and HIPAA rules for maintaining confidentiality of resident/employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.7. Prevents infections while providing care by employing adequate hand-washing, adhering to all aspects of transmission-based precautions, and complying with all infection control policies and procedures.8. Nurse Aides With Med Tech Responsibilities: Primary responsibility is to assist with Medication Administration consistent with resident support plan by identifying the correct resident, measuring vital signs if indicated by the prescriber’s order, removing medication from the original container, crushing or splitting the medication as ordered by the prescriber, placing medication in a medication cup or other appropriate container, or in the resident’s hand, mouth, or other route, and completing documentation. SPEAKINGAbility to speak clearly and effectively with residents, families, coworkers and other groups of customers. Ability to read and interpret documents such as safety rules, policies and procedures, and operating instructions. Ability to write routine reports, notes and correspondence. SEEINGVision level or accommodations sufficient to perform functions to/from/within
buildings, rooms, apartments, to complete/review required reports, notes, documentation and to use a computer. Near/far acuity and depth perception are required to operate equipment. TWISTING/BENDINGClimbing, balancing, stooping, bending and twisting, grasping and feeling, standing, sitting, walking for up to 90% day, pushing, pulling, kneeling, crouching, reaching, handling, lifting above the head.
Ability to lift up to 50 pounds or move up to 50 pounds independently. REPETITIVE MOTIONSRepetitive motions include writing, typing or entering information on the computer and hand motions that are used to work with the residents. WORKING ENVIRONMENTTemperature range from outside seasonal temperatures to inside temperatures of 65-80 degrees, varying if you work in laundry or kitchen. Each service line has a list of hazardous materials used. May be exposed to blood and body fluids, which may contain HIV and/or HBV. May be exposed to combative residents.
