Concordia Visiting Nurses is seeking a Quality Assurance RN for a full-time position which will report into our administrative office in Valencia, PA.
The Quality Assurance Nurse RN coordinates, monitors, and evaluates quality outcomes, HHCAHPs, survey results, Care Compare, and OASIS clinical data, maintaining compliance with all regulatory OASIS requirements. The Clinical Quality Review Coordinator uses communication and the resources available to promote quality, cost-effective outcomes to support the continuum of care to all patients served.

QUALIFICATIONS for the Quality Assurance RN position:
Registered professional nurse with a current valid Pennsylvania license.
Minimum of five [5] years of current experience as a professional nurse in home health care preferred
Knowledge base of agency policies, federal and state regulations and third-party payers
Exhibits strong organizational, communication and interpersonal skills
Experienced with electronic health records software and Microsoft Office
DUTIES AND RESPONSIBILITES for the Quality Assurance RN:
Performs random reviews of patient records for documentation compliance and completeness
Performs random audits to assure the agency is in compliance with all regulations related to home health care
Performs audits to assure that patients receive appropriate level of care services and appropriate utilization of those services
Conducts/Participates in Quarterly Record Review Committee meetings that include review of records, and minutes of meetings. Incorporates all information from offices into the minutes
Quality assurance activities related to infection control/surveillance for patients and employees, incident reports, patient concerns, suspected abuse/neglect (reportable events) corporate compliance, and emergency preparedness.
Respond to all Additional Development Requests [ADRs] from the Center of Medicare and Medicaid Services [CMS] or any other external audit request.
Send requests for Medicare appeals for reconsiderations, reviews, and hearings for the agencies.
Active member of the Compliance and Safety Committees for Concordia Visiting Nurses.
Participates in quarterly corporate compliance meetings.Assist with the development and updating of the annual agency emergency preparedness plan and participate in the annual hazards vulnerability assessment and ensuring completion of all annual training and testing.
Provide training and in-services for staff, as needed
Communicates all CMS updates to the staffAssist with orientation of staff in regards to Medicare requirements, PPS, OASIS, documentation requirements, etc.
Development of new forms and revision of agency forms, as needed
Provides back up to the Clinical Supervisor for the review of Plans of Care for accuracy, completeness, and signs the 485 before submitted to the physician for signature, if needed.
Provides additional education to staff as indicated
