This position is a PRN/Short-term assignment that could develop into a full-time position.
Position Summary
The Social Worker provides services to facility residents/patients and their families/responsible parties in accordance with current Federal and State regulations and facility policies and procedures in order to assure that the psychosocial needs of the resident are met/maintained on an individual basis.
Qualifications:
Bachelor’s Degree in Social Work or Human Relations field including but not limited to sociology, special education, rehabilitation counseling, or psychology.
One year of supervised social work experience in a health setting working with individuals. Ability to work harmoniously with others, both individually and as a team member.
Essential Duties and Responsibilities
Facilitates new resident admissions by obtaining a social history, completing required paperwork with residents/responsible parties (EX: PASSR), distributing paperwork to appropriate staff, completing an assessment of the resident’s cognitive, emotional, psychosocial and behavioral status
Provides information and support to residents and families at admission, throughout the resident’s stay, and at discharge
Resolves problems and complaints
Serves as an advocate for resident rights through advising residents of their rights, consulting with staff and others about resident rights, and by providing staff training on resident rights and psychosocial needs
Counsels privately with residents as needed and provides individualized interventions to assure the resident’s cognitive, emotional, social and behavioral needs are being met (EX: reality orientation, validation, reassurance, behavioral management, empowerment, advocacy, TLC, grief counseling, etc.)
Assesses residents’ current condition and needs through visits with residents, contact with responsible parties, consultation with staff, record review, and observation
Participates in regularly scheduled Care Planning Conferences with the Interdisciplinary Team, completes required documentation, and facilitates implementation of team goals
Initiates discharge plans for residents within fourteen days of admission, makes revisions as needed, provides a full update of discharge plans annually, and assists residents who are planning discharge by providing information and coordinating follow-up services
Provides family support by involving families in the care planning process, providing information on resident care, condition, relevant regulations, financial and legal issues, referral to community resources, and through counseling
Completes and maintains required forms/reports
Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
