Concordia Visiting Nurses is seeking a Transitional Coordinator for our Valencia office location. This is a fantastic entry level opportunity for anyone who is looking to learn about healthcare! The Transitional Coordinator performs various duties to support the sales/marketing team. Partners with the sales/marketing team in developing and maintaining referral source relationships. Transitional Coordinator will support the liaisons in the hospital as needed.

QUALIFICATIONS for the Transitional Coordinator Position:
Associate’s degree in business or a business-related field; BSBA Preferred
Two [2][ years’ experience in an office setting
Ability to type/data enter information in an efficient and accurate manner
Thorough knowledge and operation of computers
Skills in typing and organizational abilities.
Able to work independently, demonstrating good judgment.
Must be accurate and detail oriented.
Practices good customer service skills in all working relationships.
Desire and ability to learn.
Strong interpersonal skills, respectful, and courteous nature.
Reliable, independent means of transportation for use in community activities. Driver must have a valid driver’s license with car insurance at the limits established in the personnel policies.
RESPONSIBILITIES for the Transitional Coordinator Position:
Assists with the coordination of referrals
Data Enters patient clinical data into the current software system
Generates and distributes reports from various software systems for Clinical Management
Maintains confidentiality of patient and staff information
Handles general inquiries, whether received by telephone, mail, or in person
Coordinates homecare or post-acute skilled nursing needs to assure quality patient care and delivery of services: helps promote information transfer between levels of care.
Provides timely response to all intake inquiries at assigned institution; works closely with Transitional Nurse Liaison and Clinical management.
Identifies, develops and builds upon referral sources within assigned institution; develops relationships with other referral sources in market area.
Assists in the identification of new services that are needed or should be expanded based on experience/input at institution.
Educates markets and promotes to patients, families and referral sources the Concordia Lutheran Ministries continuum of care.
Provides education to the adult and geriatric patient/family regarding expectations of homecare plans and discharge to SNF for post-acute care.
Performs work under the supervision of the Agency Management.
Answers the phone, takes inquiries or messages using professional telephone technique.
Attends required and appropriate office meetings.
Maintains a good working relationship with all staff.
