Position Summary
The Occupational Therapist [OTR/L] is a registered and licensed qualified professional who directs, supervises, evaluates and provides occupational therapy services to patients in their home or current residence as prescribed by the attending physician.
Qualifications
- Graduate of an American Occupational Therapy Association Accredited Program of Occupational Therapy
- Registered with the National Board of Occupational Therapy
- Licensed as an Occupational Therapist in the Commonwealth of Pennsylvania
- One-year clinical experience in physical disabilities as a registered occupational therapist. Two years’ experience preferred
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a car with insurance limits as established in personnel policies
Clinical Skills
- Performs a comprehensive evaluation of patients by assessing level of function with activities of daily living
- Develops the plan of care cooperatively with the physician, patient/family and other team members with revisions based on the patient’s condition.
- Instructs patients, families and other health care team members in the total occupational therapy program, including contacting the Clinical Supervisor as appropriate when formal staff education is needed
- Assigns, instructs and supervises [every 14 days] the home health aide, in accordance with the physician’s plan of care, when the care assignment is relevant to OT needs and/or when the OTR/L is the coordinating health professional
- Serves a primary agency provider with coordination responsibilities when occupational therapy service qualifies the patient for skilled care and is the sole visiting professional
- Plans with the patient/family and other team members for the patient’s discharge from service
- Implements the therapy program established by the OTR/L, providing services to restore the patient to the maximum level of independence in daily living skills and sensory and motor function
- Assists in teaching patients and in planning discharge from occupational therapy services
- OTR/L is responsible for the supervision of the care provided to specific patients by the COTA. The COTA may not carry out the initial OT evaluation, develop the care plan, secure DR orders, or independently discharge the patient.
- Supervision is carried out by:
- OTR/L is responsible for the competency evaluation, any necessary remediation, and the performance evaluation of the COTA .
- The conference may be by telephone or in person.
- ORT/Ls re-evaluation of the patient, based on treatment response, clinical needs, functional status and goal progression.
- Reviews and co-signs COTA visit notes.
- Assigns, instructs and supervises every 14 days in accordance with physician plan of care.
