- Responsible for the initial occupational therapy evaluation and assessment of functional status, endurance, visual coordination, motor skills, environmental hazards or barriers to independent living
- Setting up and implementing the treatment plan, participating in multidisciplinary care in conjunction with the nursing team
- Reporting and recording care provided for both home health and hospice patients under the direction of the patient's physician
- Identifying equipment needs and environmental adaptations
- Educating patient, caregivers and family in rehabilitative care
- Supervising the Certified Occupational Therapist Assistant in implementing the plan of care
- Revising therapy plans as needed to enhance outcome for patients
