Under the direction of the DON and ADON, the LPN will be fully accountable for delivering medications, supervising CNAs, and ensuring that competent care and safety is consistently delivered to all residents.

PHYSICAL REQUIREMENTS OF JOB (with or without assistance):

Walking, reaching, bending, lifting, grasping, pushing and pulling, fine hand coordination, ability to hear and respond to pages, ability to understand and follow written and oral instructions and directions; ability to communicate with residents, families, personnel, vendors and consultants; ability to distinguish smells, tastes, and temperatures; and ability to remain calm under stress.

RESPONSIBILITIES

o Follow established facility policies and procedures to provide treatment to residents in accordance with physicians’ orders. Administer treatments per physician order and accurately records treatments administered. Write weekly progress note if required.

o assess and report changes in resident’s condition including development of decubitus to physician and DON. Take follow-up action as necessary.

o Assess each resident’s skin condition weekly and/or as necessary, including stage, size (measurements), sites, depth, color, drainage and odor of each condition.

o Provide emergency care to residents when necessary.

o Requisition adequate supplies to provide treatment to residents.

o Perform basic nursing skills.

o Obtain physicians’ orders and any revisions for treatments as necessary.

o Participate in supervision of care provided to residents as requested by DON.

o Record necessary information to implement appropriate follow-up action as relates to the residents’ treatment needs.

o Identify safety hazards and initiate corrective action. Identify special nursing problems and emergency situations quickly and initiate immediate action.

o Participate in facility educational programs including but not limited to, orientation, ins-service and nursing assistant education.

o Provide clinical supervision to nursing assistants if requested.

o Participate in interdisciplinary team and rehabilitation meetings as required.

o Use and supervise the use of treatment supplies and equipment efficiently and in a cost-effective manner

o Be responsible for the safety of residents under his/her supervision.

o Assess resident needs and add to resident care plan. Review and update resident care plans as necessary and as assigned.

o Follow Residents’ Rights policies at all times.

o To perform other duties as requested by management.
