Concordia Visiting Nurses is looking for a Clinical Liaison to work closely with our Washington Partners!
The Clinical Liaison is responsible to coordinate all offered services for home healthcare as applicable. This may include completing part or all of the admission process for patients. Partners with the sales team in developing and maintaining referral source relationships. Their main responsibility is discharge planning to the agency from facilities.

QUALIFICATIONS
- Associate's degree in nursing preferred or Medical Social Worker
- Case manager certification and/or specialty certification preferred.
- Maintains current PA Driver’s License and auto insurance.
- Strong clinical assessment skills.
- Skills in typing and organizational abilities.
- Preferably has home healthcare experience.
- Able to work independently, demonstrating good judgment.
- Must be accurate and detail oriented.
- Practices good customer service skills in all working relationships.
- Desire and ability to learn.
- Strong interpersonal skills, respectful, and courteous nature.
- Absence of criminal background.
