Key Responsibilities:
- Collaborate with social workers and discharge planners to ensure optimal patient outcomes
- Develop and implement patient-centered care plans
- Contribute to the development of policies and procedures related to case management
Requirements:
- Current state RN license
- Associate's or Bachelor's degree from an accredited nursing school
- Minimum 3 years of experience in acute care as a Professional Nurse or RN experience with significant acute care hospital Case Management experience
- BLS Certification
