Job Summary
The RN Case Manager will work collaboratively with the interdisciplinary team to develop individualized patient care plans and ensure payer requirements are met.
The RN Case Manager will also provide specialized interventions and community resource management to support patient success.
Requirements
Applicants should have a current state RN license and possess an associates or bachelor's degree from an accredited nursing school.
A minimum of 3 years experience in acute care as a Professional Nurse is required, with significant acute care hospital case management experience preferred.
