Family Caregiver Support Program Coordinator

Hourly Wage: $23.28-$26.20/Regular/Full-Time


Incumbent will identify the needs of unpaid family caregivers that are providing personal care for a family member or disabled adult (18 years or older). Provides individual counseling, support, and training to unpaid caregivers through home visits, phone contacts, community education, work groups, etc.; provides information and assistance/ referral services; assesses, authorizes, monitors, and prioritizes respite care/ supplemental services; initiates, develops, facilitates and /or provides technical support to support groups; conducts outreach, community education, health fairs, etc. Specializes in eligibility determination, services authorization, and case management for consumers in the Medicaid Alternative Care (MAC) and Tailored Support for Older Adults (TSOA) programs. Provides services to Kindship Caregivers.


Examples of Work Performed:

Medicaid Transformation Project (MAC/TSOA) Provides case management services to consumers receiving service through the Medicaid Transformation Project Demonstration (MTPD). Conducts semi-annual rescreens, annual assessments, and home visits, updates care plans, makes referrals, and provides ongoing support to consumers utilizing multiple database systems. Performs MTD screening to approve Housekeeping, errands, and respite services.

Family Caregiver Support Program (FCSP). Provides an advanced level of and with specialized knowledge and of practice that can be differentiated from the general social work by the ability to expand clinical knowledge in the progression, provide consultation and guidance to colleagues, role models effective social work practice skills, teach or provide orientation to less experienced social workers, and develop innovative in family caregiver's burnout interventions.

Responsible for developing and establishing linkages and collaborating on caregiver support efforts with community healthcare and social services agencies. Administratively responsible and accountable for the program development and effectiveness of and reporting responsibilities of the Yakama Nation FCSP.

Lead Kinship screening and track funds. Provides support to relative caregivers (grandparents, aunts, uncles, siblings, etc.) and the children in their care.

Provide financial assistance applications that support children in their care.

Assist with the completion and processing of financial assistance applications.

Tracking kinship caregivers and completing quarterly reports.

Meet with kinship caregivers at appropriate stores to purchase needed items (if they choose to spend the money on clothing, food, hygiene items, etc.) for children in their care.

Completes QA reporting on TCARE work submitted by other staff.

Completes and meets deadlines for NAPIS reporting. Meet contract requirements and participate in the 7.01 Area Plan. Develops WHO protocol and provides outreach to achieve milestones outlined in contracts.

Approves purchases and ensures that vendors receive payment on behalf of kinship caregivers such as power companies and summer school venues.

Complete contracts for State Family Caregivers, Respite, Housekeeping, and Errands and participate in many meetings internally and with outside agencies.

Knowledge, Skills and Abilities:

Knowledge of principles and practices of social work.

Knowledge of customer service techniques.

Knowledge of team building concepts.

Knowledge of technical program development methods and techniques.

Knowledge of terminology, law, rules, and regulations governing the area of assignment.

Knowledge of Tribal contracting principles and techniques.


Ability to adhere to confidentiality requirements of client records which includes federal, state, and tribal confidentiality policies.

Ability to communicate effectively both orally and in writing with the Aging and Long-Term Support Administration (ALTSA), government agencies, other professionals, and the general public.

Ability to establish and maintain effective interpersonal relationships with Tribal, City, and County officials, at all organizational levels and with the public.


Minimum Requirements:

Bachelor's degree in Social Work, Social Science, Counseling, or related field; OR any equivalent combination of experience, training, and/or education approved by Human Resources.

Four years of social work experience related field.

Must be able to lift and carry 50 lbs.

Must have the ability to travel within Washington State to attend required training as needed.

Must submit a driving abstract for the past 5 years at the time of application.

Required to complete the online DSHS HIPAA Privacy Training upon hire.

Required to complete DSHS TCARE Assessor Training and become certified within 3 months of hire or as soon as offered by DSHS.

Must pass a pre-employment background check.

Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving permit.

Required to pass a pre-employment drug test.


Preferred Requirements:

Bilingual English/Spanish preferred, not required.

Enrolled Yakama Preference, but all qualified applicants are encouraged to apply.
