The Wraparound Care Coordinator will be responsible for assessing and connecting children, youth, and their families who have been identified by the behavioral health clinical team to a continuum of coordinated mental health treatment and wellness services. The Wraparound Care Coordinator will be responsible for completing a whole-person needs assessment that determines the child's, youths, or family member's social determinants of health and for developing a family-driven and youth- guided care plan that addresses needs across life domains. This role requires familiarity of community services because of the need for managing communication and coordinating services between agencies necessary to help children, youth, and their families get the services they need.
The Wraparound Care Coordinator plays a pivotal role in coordinating comprehensive, client-centered services to individuals and families who need support accessing and navigating a broad range of healthcare, social services, and community resources. This position involves working directly with clients, service providers, and community organizations to ensure that clients receive personalized, holistic care and support designed to meet their specific needs. The Wraparound Care Navigator serves as a guide throughout the process, advocating for clients while fostering collaboration across service systems to achieve improved health and well-being outcomes.
Examples of Work Performed :
Client Engagement and assessment : Conduct thorough assessments of clients' needs, strengths, and challenges to develop individualized care plans.
Build trust and rapport with clients and their families, ensuring they feel supported and heard throughout the process.
Help clients articulate their goals and assist in navigating systems that support mental health, substance use treatment, education, housing, employment, and healthcare.
Assist in development of the intake assessment, preliminary diagnosis, ongoing brief, and community support services. Including case management, to identify children, families, and adults in a culturally competent manner within the most appropriate environment. Must maintain a minimum of 50% work hours in billable services hours.
Care Coordination : Develop and implement "wrap-around" care plans that address multiple facets of a client's well-being, ensuring coordination of services.
Develops and implements treatment plans which focus on client strengths, needs, and goals for successful achievement.
Connect clients with appropriate resources, including medical, behavioral health, housing, transportation, legal services, and community programs.
Work closely with other service providers (e.g., social workers, healthcare providers, case managers) to ensure seamless service delivery and reduce service fragmentation.
Provide services, assessments, individual, family and group services for all required populations such as children, adolescents, adults, elderly, LGBTQ, and veterans, in a variety of settings including office, HIS clinic, local Hospitals, Tribal Court, Tribal Corrections, homes, and some off reservation settings.
Advocacy and Support : Act as a client advocate in interactions with service providers, ensuring that clients' and respected and that they receive services in a timely and culturally competent manner.
Help clients navigate complex systems such as healthcare, government assistance programs, legal services, and educational institutions.
Monitoring and follow-up : Regularly monitor client progress and ensure that care plans are updated as needs change.
Provide ongoing follow-up and support to clients, ensuring they stay engaged with services and are achieving their care goals.
Document and track follow-up services provided, and outcomes achieved, ensuring compliance with organizational standards and regulations.
Collaboration and Teamwork : Work as part of a multidisciplinary team to ensure the coordination and delivery of wrap-around services.
Attend case conferences, team meetings, and other collaborative efforts to review cases and share updates on
Client progress.
Build and maintain strong relationships with staff, local community organizations, service providers, and stakeholders to ensure resource availability.
Participates in Supervisory, Multi-Disciplinary Team Staffing, training, local Advisory / Coalition meetings, and child protective meetings.
Establish and maintains working relationships with other tribal programs as well as non-tribal programs and
resources that pertain to YNBHS patients and families.
Cultural Competency : Provide culturally competent care that respects the diversity of clients' backgrounds and
needs.
Use a trauma-informed approach in all interactions to support clients' emotional and psychological well-being.
Data Entry and Reporting : Maintain accurate records of client interactions, care plans, referrals, and progress reports using grant and organizational data systems.
Contribute to the tracking of program outcomes and performance metrics.
Professional Development :
Stay informed about changes in laws, regulations, and best practices related to wrap-around services and client care.
Participate in ongoing training and professional development activities.

Knowledge, Skills and Abilities :
Knowledge of the basic principles, practices, and professional standards of providing clinical behavioral / mental health services to tribal communities.
Knowledge of completing documentation not limited to treatment plans, assessments, and service authorizations.
Ability to complete billing in departments electronic health records software in a timely manner.
Ability to make professional evaluations, decisions, and recommendation for treatment planning and implementation.
Knowledge of community agencies and services provided.
Knowledge of HIPAA laws and how it pertains to Mental Health policies.
Ability to navigate service system and to ensure that children, youth, and their families are connected to services.
Knowledge, skills, and ability to help Yakama children and families by providing services in ways that respect Yakama culture and spiritual practices.
Proficiency in Microsoft Office Suite and experience with case management or client tracking software.
Ability to provide services in the Yakama context of extended family and community involvement.
Ability to handle multiple projects at the same time in the context of a quality improvement program.
Ability to influence and facilitate community and group decision-making processes through knowledge, ideas, and persuasion.
Strong interpersonal skills, with the ability to build rapport and trust with clients from diverse backgrounds.
Ability to navigate and coordinate services across multiple systems (e.g., healthcare, social services, education).
Excellent communication skills, both verbal and written.
Strong problem-solving and organizational skills, with the ability to manage multiple cases simultaneously.

Minimum Requirements :
Bachelor's Degree in Education, Social Work, Psychology, Nursing, and / or Related Counseling field, or 1 year of related clinical experience working with children, families and other individual's psychosis and / or other behavioral health issues.
Must have or be eligible to apply for associate / affiliate status through Washington State Department of Health. Full licensure must be acquired within the time frame established by the Department of Health and Director.
Must maintain strict confidentiality.
Must pass a pre-employment background check.
Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving permit.
Required to pass a pre-employment drug test.
Preferred Requirements :
Two years of Case Management experience.
Enrolled Yakama Preference, but all qualified applicants are encouraged to apply.
