Job Description

SUMMARY:

The Public Health Nurse will lead medical services for the Public Health Department, including medical triage, care management, education, provider support, vaccination, and monitoring health indicators like environmental illnesses, immunization rates, mortality, and communicable diseases to develop interventions. Additional responsibilities include patient referrals, home visits, individual and group counseling, health promotion, illness prevention, health assessments, and policy development. This role collaborates with government agencies, health administrators, and other Tribal Health workers. The Public Health Nurse will also supervise new staff, such as Licensed Practical Nurses, Certified Nurses Aides, and Public Health Administrative Assistants.

Notice: This position is part of the Public Health Modernization Grant - this is a full-time limited duration grant-funded position. Funding is through 6/30/2027 but may continue from that point depending on further extension or state funding of the program.

Principal Activities & Responsibilities

  • Collaborates in the development and delivery of programs and activities for individuals, families and population groups that promote health and prevent disease, in settings including, but not limited to homes, community organizations, businesses, schools, and community events.
  • Participates in the assessment and monitoring of population health using systematic, evidence-based processes to solve community health problems.
  • Ensures the delivery of communicable disease services including screening, case investigations, referral for treatment, and contact tracing in alignment with Oregon Health Authority recommended investigative and treatment guidelines.
  • Provides preventative and intervention education and counseling while using appropriate and professional methods of communication to individuals, families and community groups that are adapted to their unique needs, lifestyles, physical abilities, sexual orientations, cultural and socio-economic situations, and racial and ethnic backgrounds.
  • Works closely with the primary care providers teams to coordinate care, ensure consistent communication concerning patient health status, care plans, and outcomes of interventions.
  • Supervises subsequently hired Public Health Department personnel.
  • Maintains confidentiality in accordance of the CTCLUSI Employee Handbook, policies and HIPAA.
  • Develops Policies and Procedures Manual in accordance with the Public Health Association Board Accreditation standards for the Public Health Department and annually review and update said manual.
  • Represents CTCLUSI with a respected and viable presence at community events and while in attendance at conferences, meeting, courses and events outside of the CTCLUSI boundaries.
  • Actively participates in the monitoring of the high-risk elder care list via monthly meeting and ensure appropriate individuals are represented through advocacy and communication with all parties/entities vital to the safety and care of the elder population.
  • Provides home visits per policy as ordered by primary care providers and accordance with documented care plans.
  • Participates in the planning of health education campaigns and disease prevention activities that involve improving access among community members to clinical care services.
  • Performs public health emergency response and preparedness duties as assigned and consistent with job classification and training provided.
  • When assigned case management, assesses, implements and evaluates patient status using the standard of nursing care and appropriate delegation of tasks to Community Health Representatives.
  • Assists Public Health and Medical teams with immunization management and coordination, providing immunizations to community members of all ages upon request, by appointment, or during immunization clinics.
  • Delivers targeted, culturally-appropriate information to help individuals, groups and entities understand local environmental public health policies, regulations, and Tribal, state and county codes.
  • Assists in the design, production and dissemination of collateral marketing materials (letters, brochures, Tribal newsletters, and flyers) in accordance with CTCLUSI policies.
  • Adheres to ethical principles and CTCLUSIs policies in the collection, maintenance, use, and dissemination of data and information.
  • Uses quality improvement (QI) and performance management (PM) processes and/or techniques to improve the effectiveness of the respective public health program. This includes, but is not limited to: creating, implementing and evaluating performance standards and identifying, implementing and assessing program quality improvement processes.
  • Provides monthly reports to appropriate departments, management team and government entities as needed and requested.
  • Participates in the quarterly Northwest Portland Area Indian Health Board (NPAIHB) Public Health Modernization Community of Practice Gatherings.
  • Educates the public about health issues and empower them to aid in the prevention of spreading diseases.
  • Develops and provides the community with health materials (i.e., posters, infographics, pamphlets, monthly newsletter articles), while communicating effectively and with compassion, to educate the target audience, and make sure that they fully understand the information provided.
  • Works as part of the health team to coordinate activities and interventions of other Tribal, State and Federal health and social service agencies to assist Tribal members and their families.
  • Interacts with Tribal members and the general public in a courteous, professional, and efficient manner.
  • Complete other duties as directed by management.

  • This is a supervisory position.

  • Must be able to walk, talk, hear, use of hands to handle, feel or operate objects, tools, or controls, and reach with hands and arms.
  • Vision abilities required by this job include close vision and the ability to adjust focus.
  • May be required to push, pull, lift, and/or carry up to 40 pounds.

WORKING CONDITIONS AND ENVIRONMENT

  • Moderate noise level with frequent interruptions and distractions.
  • May require local and out of town/overnight travel.
  • May be required to attend Tribal youth camps and other activities.

Required Experience

MINIMUM JOB REQUIREMENTS

  • Must be at least 25 years of age per Tribal Vehicle Use Policy.
  • Registered nurse with BSN Degree and training in Public or Community Health Nursing required.
  • Valid drivers license, adequate transportation, and eligibility to operate GSA vehicles required.
  • Experience and/or knowledge of history, culture and health needs of Native American communities is preferred.
  • Must have or obtain within 90 days of hire a Food Handlers card. Re-certification must be maintained as required thereafter.
  • Must have or obtain within 90 days of hire First Aid and CPR certification (or as soon as the training is available if there is no training scheduled in the 90-day window). Re-certification must be maintained as required thereafter.
  • Advanced user in Microsoft Office Package (Access, Excel, Word, Publisher, PowerPoint, Teams, Zoom).
  • Excellent verbal and written communication skills are required.
  • Must be highly organized and motivated and be able to manage complex projects and carry out all responsibilities of the job requirements with minimal day-to-day supervision.
  • Must be able to travel, as requested.
  • Must possess ability to work with a wide variety of people and the ability to communicate effectively and relate to Tribal members.
  • Must possess reasonable ability to communicate in English.
  • This is a covered position and will be subject to pre-employment drug testing and criminal history background check, which will include fingerprinting.
  • Must be eligible for employment in U.S.
  • Indian preference will be observed in the hiring process.
