Starting Wage Range: $27.05/hr. - $34.95/hr.
Job Summary:
Responsible for the daily planning and operation of grant deliverables and reporting. Coordinates and assists with the facilitation of dementia-friendly community efforts, and supports people with dementia and their family members. This is a two-year grant-funded position.
Essential Functions:
- Promotes and provides culturally appropriate care, events and activities that improve connectedness to the Tribe.
- Provides culturally specific information, support and referrals related to the identified needs of clients. Assists with the development and maintenance of client referrals and brochures. Collaborates with various organizations for program development.
- Prepares and conducts community presentations to increase knowledge of dementia and other memory loss conditions for tribal programs and services.
- Conducts individual client and group meetings.
- Provides information, support, and referrals related to the identified needs of clients.
- Assesses individual/family needs; identifies and coordinates the delivery of services.
- Assists with compiling necessary data for grants.
- Attends meetings and serves on committees as directed by manager.
- Participates in case management conferences/meetings with Medical and Community Health team members to ensure the exchange of information concerning client care, transports, and coordination of preventive health activities is accurate.
- Carryout plans for dementia-specific technical assistance related to cognitive screening, individual and family caregiver issues and assists with various related resources in various community locations community-wide; maintains and promotes cultural and traditional teachings at all times.
- Facilitates dementia-friendly community efforts, delivers dementia and program services training, and support community members with dementia and their family.
- Conducts patient assessments, screenings and evaluations.
- Promotes communication and collaboration among the dementia healthcare teams and with various organizations for program development.
- Adheres to HIPAA law as it impacts a client’s private health information.
- Maintains accurate and current client records and program reporting as required, including documentation in electronic health record and shared department folders.
- Performs other grant related activities and a variety of other duties as assigned by the manager.
Education, Experience, and Licenses:
- Requires a High School diploma or GED.
- Requires Certified Medical Assistant, license/certification.
- Requires two (2) years’ experience as a CMA in a clinical care setting.
- OR equivalent combination of education, training, and experience.
Driving Requirements:
- Must maintain a valid Oregon driver’s license or obtain one upon hire, qualify for the Tribe’s vehicles insurance, and provide proof of personal vehicle insurance.
Qualifications, Registrations, and Certifications:
- Requires ALZ training and certification through Alzheimer’s Association preferred, OR completed within 3 months of hire.
- Obtains Traditional Health Worker certification through the Oregon Health Authority within 18 months of hire.
- Self-motivated and proactive to accomplish goals with a strong sense of accountability for results.
- Requires knowledge of the Privacy Act and its application.
- Requires First Aid/CPR certificate within 3 months of hire.
- Requires knowledge of disease prevention and basic caregiver techniques.
- Requires demonstrated experience with performing health screenings to include operation of blood pressure monitors, thermometers, and blood glucose machines.
- Must have working knowledge of HIPAA law.
- Must have ability to organize and coordinate multiple activities.
- Must have knowledge of Microsoft products, NextGen electronic health records, phone, copy, and fax machine.
- Knowledge of and sensitivity to Native American culture is required.
Environmental Factors and Conditions/Physical Requirements:
- Continually requires the ability to express or receive detailed information or important instructions through oral communication.
- Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers. Often requires walking or moving about to accomplish tasks.
- Must have the ability to work at times with minimal supervision.
- Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally.
- Occasionally requires lifted of up to 40 pounds.
- This position is frequently subject to inside environmental conditions which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions.
- Due to the nature of interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations.
- May be called upon to serve as a Disaster Reservist during emergencies.
- Maintain proper physical and mental health and condition to overcome adversities of the conditions faced in response to emergencies.
- Local travel is occasionally required. Travel outside of the state is infrequently required.
