INTRODUCTION:
The incumbent shall be sensitive to the diversity of Native cultures as well as have knowledge of the disease of alcoholism/substance abuse issues and familiarity with the harm reduction model of care. In collaboration with the MAT team, this position provides comprehensive medication assistance treatment (MAT) case management to 75-150 patients enrolled in the program. The multidisciplinary MAT team includes medical providers, mental health counselors, A&D counselor, and peer support. The team works together to support patients in recovery from opioid addiction.

DUTIES:
  • Facilitate initial intake of potential clients including but not limited to: registration and insurance verification/ prior authorizations, physical exam, labs, and medical history.
  • Monitoring client participation via chart review, order tracking, prescription monitoring, lab review, group attendance. Notify MAT team when concerns noted.
  • Assist clients to schedule follow-up visits with MAT PCP, counselor and other healthcare partners. Assist in arranging transportation if required.
  • Work with peer support specialist, peer mentors, and health navigator to find Tribal and community resources based on individual client needs.
  • Manage counselors’ schedule, manage group attendance, and send invites for virtual groups.
  • Facilitate induction to include: ensuring all pre-induction requirements met, consents signed, and ROI’s in place. Perform physical assessment, prior to and throughout induction process. Monitor vital signs. Monitor for signs and symptoms of precipitated withdrawal. Document induction process. Schedule Induction day 2 follow-up.
  • Medication prior authorization - complete forms and track authorization. Work with specialty pharmacies to obtain specialized medications when ordered by MAT providers.
  • Collaborate with team to maintain accurate patient data including a comprehensive list of measures tracked by the program to support patient care.
  • Trace case information from start, throughout program participation for MAT clients quarterly, bi-annually and annually in order to generate grant reports for grant funding.
  • Track data in various health systems to ensure grant reporting requirements are met.
  • Facilitate MAT case management meetings. Prioritize agenda, keep meeting notes, and assign tasks as result of care team meetings.
  • Ensure program compliance with policies and procedures including revisions and implementation of new policies.
  • Other duties as assigned.

Requirements

REQUIREMENTS:
RN licensure in the State of Oregon with an interest and knowledge of harm reduction and medication assistance treatment program.

Knowledge of:
  • Medications, including potential side effects and interactions.
  • Medical records, medical procedures, and terminology.

Ability to:
  • Recognize symptoms and provide recommendations within scope of practice.
  • Administer prescribed treatments, perform procedures, and carry out tasks ordered by providers within scope of practice.
  • Respond quickly and appropriately to situations that may arise.
  • Ability to use critical thinking skills to adjust actions in order to provide patient centered care.
  • Adapt to rapidly changing situations.
  • Use effective written and oral communication skills to explain complex medical issues, work with team members and providers, and discuss difficult or sensitive issues.
  • Listen and understand complex information.
  • Be an effective member of a multi-disciplinary care team.
  • Work, communicate, and contribute to cooperative working relationships with a wide variety of co-workers.
  • Use scientific and psychosocial nursing principles, practices, techniques, and processes to provide care from infant to geriatric.
  • Read, interpret, and apply policies and procedures along with applicable laws and regulations; including, Tribal, Federal, State, and local jurisdictions.
  • Understand and work within tribal structure, culture, and values.
  • Work with diverse populations. Possess an understanding of disease process and communication needs for various age ranges.
  • Apply personal sensitivity to the needs and experiences of others. Provide non-judgmental care towards all persons, utilizing the harm reduction model.

Other:
  • First Aid and CPR or must acquire within 30-days of hire.
  • Must possess a valid driver's license, insurance and be eligible to drive a GSA vehicle.
