The MDS Coordinator is responsible for implementing the federally mandated assessment tool to identify the concerns of the residents and to coordinate a care plan to assist the residents toward optimal wellness based on the highest practicable level of functioning.

- Manage the overall process and tracking of all Medicare/Medicaid case-mix documents to assure appropriate reimbursement for services provided within the nursing center.
- Conduct concurrent MDS reviews to assure achievement to maximum allowable RUG categories.
- Integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement.
- Perform other job-related duties as they become required.
- MDS/RAI experience. Skilled Nursing Experience Preferred.
- Must have Licensed Nurse Practical (LPN) or Registered Nurse (RN) certification in Ohio.
- Knowledgeable of all Medicaid legal regulations, guidelines, and best practices.
- Demonstrated skills in Microsoft Word and Excel. Power Point and Outlook, Internet navigation.
