Central Admissions Director – Skilled Nursing Facilities (Northern Indiana)
We are seeking an experienced Central Admissions Director to manage the patient referral and admissions process for our skilled nursing facilities in Northern Indiana , including Chesterton, Crown Point, Plymouth, Logansport, Kendallville, Warsaw, and Elkhart.
About the Role
As the Central Admissions Director , you will play a vital role in ensuring a smooth transition for patients entering our skilled nursing facilities. This position requires strong clinical knowledge, insurance expertise, and excellent communication skills to coordinate admissions effectively.
Key Responsibilities
Develop and maintain relationships with referral sources, including hospital discharge planners, case managers, and physicians.
Manage inquiries and referrals, ensuring a timely and professional response.
Conduct clinical assessments to determine admission eligibility and communicate patient needs to facility teams.
Obtain prior authorizations and precertifications for managed care admissions.
Oversee the admissions process, including bed availability tracking and coordination of necessary equipment.
Monitor and analyze referral trends, identifying opportunities for growth and improved patient flow.
Work closely with facility leaders and regional teams to optimize occupancy and payer mix.
Collaborate with sales teams to promote facility services and educate stakeholders on the admissions process.
Maintain accurate tracking of referrals and admissions in PCC and manual logs.
Qualifications
Candidates should have two or more years of experience in the skilled nursing industry in one of the following roles :
