MDS Coordinator - Skilled Nursing Facility
We are seeking a skilled MDS Coordinator to join our team in a fast-paced and rewarding environment. The MDS Coordinator plays a critical role in ensuring that residents receive the highest quality care by conducting assessments, ensuring compliance with regulatory standards, and coordinating care planning processes. As the MDS Coordinator, you will be responsible for completing the Minimum Data Set (MDS) assessments for residents, coordinating care planning meetings, and ensuring that the care provided aligns with the patient's needs and regulatory guidelines. You will work closely with the interdisciplinary team to ensure that accurate and timely information is communicated and utilized in developing comprehensive care plans.
Key Responsibilities :
- Complete MDS assessments for all residents in compliance with federal and state regulations.
- Collaborate with interdisciplinary team members, including nursing, physicians, and social services, to develop and update care plans.
- Ensure the timely and accurate submission of the MDS assessments to the appropriate regulatory agencies.
- Monitor and track resident outcomes, identifying areas for improvement and suggesting quality improvement strategies.
- Provide education to staff and families regarding care planning and the MDS process.
- Ensure that the facility is in compliance with all regulations regarding assessments, care plans, and documentation.
- Assist with the development and implementation of individualized care plans based on MDS findings and interdisciplinary team input.
