CARE MANAGER – SOCIAL WORK (MSW)
We have a great opportunity for a Care Manager to join our pediatric practice on a full-time basis. This position is 4 days per week (32 – 40 hours). Care Management responsibilities include partnering with patients to optimize their ability to self-manage their behavioral health conditions, identify and develop care plans and assist with care coordination. Ideal candidate will have strong communication skills, ability to multi-task and previous Care Management experience. Current, unrestricted Michigan License required.
Pay starting at $28/hour depending on experience.

JOB DUTIES FOR THE MSW CARE MANAGER:

QUALITIES AND REQUIREMENTS FOR THE MSW CARE MANAGER:
- Excellent time management and organizational skills
- Current, unrestricted Michigan License (LMSW)
- Previous Care Management experience
- Strong communication skills
- Basic - intermediate computer skills
