Overview
We are seeking a dedicated and knowledgeable Certified Diabetes Care and Education Specialist(CDCES) to join our healthcare team. The diabetes educator is fully integrated in a team based in Endocrinology. This role is essential in providing comprehensive Diabetes Self-Care Management Education (DSME) to patients and families as part of a collaborative multidisciplinary team in a clinical role. The ideal candidate will possess a strong background in nutrition education training insulin pumps, meters, and continuous glucose monitors to enrich the patient experience specific to people living with diabetes. Health coaching and teaching, with a focus on developing personalized care plans that promote weight loss for patients with multiple comorbidities and overall health improvement.
Duties
- Provide diabetes education following American Diabetes Association and Association of Diabetes Care & Education Specialists guidelines via in-person appointments, incident to office visit appointments, virtual telehealth appointments, and diabetes support groups.
- Develop and implement individualized care plans that address dietary needs, assess patient for medication adherence regimens and device options.
- Collaborate with healthcare team to ensure integrated care for patients, preferred experience with billing for dietician/nutrition services.
- Provide health coaching to patients and families to promote understanding of diabetes management and healthy lifestyle choices.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Educate patients about diabetes management, including the importance of diet, exercise, medication adherence, and nutrition education.
- Maintain accurate documentation of patient assessments, care plans, and progress notes in compliance with healthcare regulations.
Experience
- 1 year required as a specialized diabetes care as a licensed dietician or nurse.
- Strong knowledge of Diabetes Education Specialist/Diabetes Educator with an RD/LD or RN credentials, may assess and provide nutritional care to patients.
- Background in nutrition education with an emphasis on diabetes management is highly desirable.
- Provide visual and hands-on education on continuous glucose monitors, insulin pumps, blood glucose monitors, and medications.
- Demonstrated ability to create effective care plans tailored to individual patient needs.
- Excellent communication skills to effectively educate patients and collaborate with healthcare teams.
