Position Summary:
Provides nutritional care for residents in accordance with the facility policies and procedures and as consistent with guidelines developed by the New York State Department of Health, and Federal governing bodies of long-term care facilities. Collaborates with members of the health care team to provide nutritional care. Identifies residents at nutrition risk based on criteria outlined by the Dietary Department. Assesses nutrition needs and identifies nutrient requirements according to specific needs of residents. Develops, implements, monitors, and evaluates the nutrition care for all assigned residents. Collaborates with the food service team in integrating the residents’ nutritional needs with the services of the dietary department.

- Accurately and appropriately determines the nutritional status of residents through completion of nutritional assessments based on facility policies and in accordance with time frames outlined in facility policies. Completes all RAI (Resident Assessment Instrument) requirements as per facility policies.
- Appropriately determines the nutritional and hydration risk level of residents.
- Assesses the adequacy of calorie, macronutrient, and fluid intake within facility guidelines.
- Determines nutrient requirements based on department criteria.
5. Actively collects, reviews and assesses anthropometric data, biochemical parameters (eg, sodium, potassium, albumin, blood urea nitrogen), past and current medical and social history and food intake to determine the nutritional needs of residents.
6 .Collaborates with the resident and/or family member, and health care team to assess nutrition and nutrition education needs of patients.
7. Identifies need for nutrition supplementation for residents.
- Reviews, collects, inputs in computer, and assesses monthly and/or weekly weight data.
9. Includes religious, cultural, and ethnic factors, as well as residents’ wishes, when completing nutrition assessments.
10. Develops meaningful and individualized nutritional interventions based on the assessed needs of the residents and documents in the care plans.
11. Provides timely follow-up monitoring and evaluation of residents identified to be at nutrition risk.
12. Identifies appropriate tube feeding formula product within the facility formulary to meet the resident requirements.
13. Develops appropriate interventions to maintain or improve the nutritional status of the resident.
14. Continuously monitors and evaluates the nutrition care and outcomes of residents.
15. Evaluates and changes nutritional interventions as appropriate and when they are ineffective.
16. Develops appropriate and individualized goals for residents.
17. Determines nutrient requirements based on departmental criteria.
- Actively and appropriately participates as a member of the resident care team including care plan meetings and discussions of resident care. Respond to consults, questions, and discuss resident nutrition related problems with appropriate members of care team
- Provides nutrition education to residents and/or family members as required.
- Provides substitute foods of similar nutritional value to residents.
- Disseminates resident diet orders, food preferences, and related data accurately and in a timely manner to the diet aides.
- Maintains accurate computer records related to resident’s diet order, food preferences, food allergies, snacks, room number, unit, meal location, weight status, and any other needed.
- Is knowledgeable of therapeutic diets and demonstrates the ability to use the facility’s diet manual.
- Monitor residents during mealtime to ensure proper meal service and satisfaction.
25. Uses EMR system effectively to input appropriate pending orders.
26. Uses effective interpersonal communication skills in providing for resident food services.
27. Actively communicates accurate resident nutrition status/needs to appropriate team members.
28. Demonstrates competency in outcomes tracking through documentation in progress notes.
29. Performs other related duties as assigned.
