Job Summary
The Home Healthcare Coordinator is responsible for overseeing and managing the delivery of home healthcare services to patients. This role involves coordinating care plans, managing patient schedules, liaising with healthcare professionals, and ensuring compliance with healthcare regulations. The Home Healthcare Coordinator plays a key role in ensuring that patients receive high-quality, compassionate care in the comfort of their homes.
Hours
- Monday-Friday: 8am-12pm
- On-call availability: 6am-6pm daily
Key Responsibilities
- Care Coordination: Monitor the delivery of personalized care plans for clients, ensuring they meet the medical, physical, and emotional needs of each individual.
- Patient and Family Communication: Serve as the primary point of contact for clients and their families, providing updates on care progress, addressing concerns, and answering questions about home healthcare services.
- Scheduling & Staffing: Schedule new clients with existing caregivers and new caregivers with existing clients. Adjust schedules as needed to accommodate client needs.
- Collaboration with Home Care Team: Work closely with Nursing Supervisor, Recruiter, and Operations Manager to ensure that all aspects of the client’s care and caregivers schedules are maintained.
- Compliance & Documentation: Ensure all caregiver documentation, client records, and care plans are accurate, up-to-date, and comply with healthcare regulations (HIPAA, Medicare, Medicaid, etc.). Email case managers about any changes in client care or scheduling.
- Quality Assurance: Monitor client progress, assess care quality, and advocate for improvements where needed. Ensure that clients receive the highest standard of care in accordance with company policies and industry best practices.
- Problem Solving & Conflict Resolution: Address and resolve any issues or conflicts between clients, caregivers, or family members. Handle complaints professionally, with a focus on finding satisfactory solutions.
- Support & Oversight: Provide support to home healthcare staff, taking calls and answering texts. Ensure caregivers show up to work, clock in and out, turn in visit slips, turn in credential paperwork, complete annual continuing education.
- Onboarding: Proctor written pre-employment tests for candidates and scan into system. Create and mail out new employee packets and name badges.
Experience
- Experience in home healthcare coordination, nursing, or healthcare management not required but a plus.
- Experience with patient scheduling, medical recordkeeping, and compliance regulations not required but a plus.
Skills
