Physical Therapist (PT) / Physical Therapy Assistant (PTA) – Home Health (Per Diem)
Location : Boca Raton, FL
Employment Type : Per Diem
We are seeking a Per Diem Physical Therapist (PT) or Physical Therapy Assistant (PTA) to join our Home Health team in Boca Raton, FL . This role offers a flexible schedule and the opportunity to provide high-quality rehabilitative care in a home setting.
Responsibilities :
- Conduct in-home patient evaluations and implement individualized treatment plans ( PTs ).
- Assist in executing prescribed therapy programs under the supervision of a licensed PT ( PTAs ).
- Provide therapeutic exercises and interventions to improve mobility, strength, and overall function.
- Educate patients and caregivers on home exercise programs and safety measures.
- Collaborate with an interdisciplinary team to ensure optimal patient outcomes.
- Accurately document patient progress and treatment plans in an EMR system.
- Maintain compliance with state, federal, and agency guidelines.
