Position Overview:
We are seeking a dedicated Nurse Practitioner (NP) to join our dynamic team in delivering exceptional outpatient primary care to seniors. As a key member of our collaborative team, you’ll work closely with other healthcare professionals to provide top-tier care while having the autonomy to focus on what truly matters – improving the lives of your patients.
Compensation and Benefits:
- Competitive Salary Range: $125k - $130k (based on experience)

Key Responsibilities:
- Provide outpatient primary care services to a panel of senior patients in a supportive, team-oriented environment
- Conduct a range of in-office medical procedures, including suturing, joint injections, skin biopsies, wound care, and preventive screenings tailored to geriatric populations
- Lead patient care coordination and collaborate with multidisciplinary teams to ensure comprehensive, holistic care
- Ensure quality outcomes by delivering personalized care that addresses the physical, social, and emotional needs of each patient
- Maintain a manageable caseload, seeing an average of 10-12 patients daily, with a patient panel cap of 650
- Stay current with clinical guidelines and best practices in geriatric care
- Foster a culture of patient-first care by being readily available to address patient concerns and medical needs
Candidate Requirements:
- Licensed Nurse Practitioner with at least 2 years of outpatient primary care experience
- Proficiency in performing key medical procedures, including:
- Suturing for minor wound closures
- Joint injections for pain management
- Skin biopsies for diagnostic purposes
- Comprehensive wound care management
- Preventive screenings relevant to geriatric care
- Training in Geriatrics is a plus but not required
- Active, non-probationary state medical license
- A passion for patient-centered care and the ability to work collaboratively with a diverse, multi-disciplinary team
- Commitment to working with senior populations, especially those with comorbid conditions, chronic illnesses, and medically underserved groups
- Ability to deliver compassionate care with a strong focus on building long-term relationships with patients and addressing their medical and social needs
