- Social Service Designee or BSW with Long Term Care Knowledge and Experience
- Positive, professional, outgoing personality.
- Demonstration of values, ethics, emphasizing resident empowerment and self-determination
- Proficient in interdisciplinary care planning, MDS functions, assessment and documentation as required by regulatory agencies.
- Experienced Medicare, Medicaid & Insurance liaison; assisting residents and families in receiving maximum benefit from these health resources.
- Responsible for strengthening communications among residents, families, and staff.
- Must assist residents in maintaining the highest quality of life and independent functioning possible.
- Promoting facility–community interaction by encouraging community involvement in the facility along with resident and staff involvement in the community, developing linkages with a wide range of community resources, and participating in the assessment of and the need for planning related to other long-term social and health care resources.
- This person is responsible for the coordination of services for our resident from admission to discharge!!
Job Type: Full-time
Pay: $19.00 - $23.00 per hour
Expected hours: 40 per week
