Job Description:
The Therapeutic Case Manager reports directly to the Executive Director and works under the support and supervision of the Executive Director.
The Therapeutic Case Manager is responsible for working with participants at Arete and implementing the Arete Transitions coaching program with these participants. The Therapeutic Case Manager is capable of working independently and ensuring that Participants are engaging with their goals, responsibilities, programming, and life outside of Arete in healthy and supported ways. Work includes program development and growth through administrative tasks, proposing and implementing new programming opportunities, taking on projects and completing them, managing the responsibilities of Arete Life Skills Instructors, completing paperwork in a timely manner, having weekly contact with the families of participants, coordinating and staying connected with the Participants entire team (clinician, family, life skills instructors, program directors, and workshop instructors) to ensure that the full spectrum of services are being provided and that the participant is engaging in all that are necessary.
The Therapeutic Case Manager position involves direct service with program participants, aligning the delivery of services with the goals and objectives of the participants, their families, and Arete Transitions. Additionally, the position involves a significant amount of time developing and planning educational curriculum, seasonal plans, scheduling, documentation of direct work with participants, supervision notes of staff, program policy writing, identifying and addressing challenges existing within the programming, and presenting fresh ideas and services that would benefit the Arete community and participant experience.

Responsibilities:
Participant Direct Support:
- Lead two coaching sessions with each participant (caseload) per week.
- Lead a weekly Family check-in with each participant (caseload) per week.
- Communicate directly with Life Skills Instructors, Therapists, counselors and external care providers (as needed) to organize the needs and goals of each participant (caseload).
- Assess challenges that arise among the Arete program, schedule, policies, participants, and staff and facilitate solutions.
- Ensure that weekly and daily schedules, logistical needs, appointments, and program details are up to date and communicated to all necessary team members and Participants.
- Direct and support the daily schedule and program operations for participants needs and benchmark expectations.
- Expenses and Budget management:
- Assist participants with personal budgets.
- Maintain program budget reports for expenses and activities,
- Submit budget reports to the Program Director.
- Uphold all Arete Transitions program expectations with participants, family members, and team members.
- Maintain program expectations regarding:
- Curfew
- Technology Use
- Personal Finances
- Personal Vehicles
- Guests at Arete
- Home Visits
- Advocacy Board
- Benchmark Progressions
- Participate in program activities and community events and role model a “Do With” approach and attitude.
- Plan and Organize recreational and social activities and trips:
- Rec. Leagues
- Day Trips
- Occasional overnight trips
- Weekend and evening activities and events
- Support and coach participants in reaching their benchmarks.
- Understand each Arete Benchmark and it’s expectations, responsibilities, and privileges.
- Assist participants with problem solving, motivation, and engagement.
- Connect with all participants regularly, maintain healthy relationships and boundaries, and seek opportunities to be caring and direct with guidance and expectations.
- Support with Participant Family visits to Arete Transitions.
- Assist in managing Arete Apartments, role modeling to Participants how to care for their housing needs.
Administrative and Programming:
- Complete Coaching reports after each coaching session.
- Complete Family Check-in reports after each family check in call.
- Read shift notes and updates from team members about their work with participants.
- Prepare Benchmark Reviews for each participant when they are ready to advance to the next benchmark, present relevant information during the Benchmark Advocacy Board meeting.
- Assist in the daily tasks and challenges of the Participants and Program needs, errands, care and upkeep of residences and office space.
- Assist in the program tasks related to Admissions, Team Coordination, Apartment Checks/Upkeep, Whole Person Wellness Plans, and Discharges/Graduations.
- Design and implement curriculum in support of the 9 Pillars of Whole Person Health model.
- Design new programming opportunities for Arete Participants and staff.
- Create in-depth knowledge with Google Suite (Email, calendar, chat, drive, etc) to organize and manage documents and paperwork.
- Maintain working knowledge of the daily, weekly, Participant, and Staff calendars.
- Uphold and assess program expectations for participants and staff, to regularly assess the quality of services, organization, connection to Arete Mission, and safety.
- Assist the Program Director(s) in team member trainings and regular reviews of Arete Employee Manual and Policies and Procedures.
Training/Development/Supervision:
- Seek opportunities for external training and certifications.
- Attend the weekly staff meeting.
- Attend a supervision meeting once a week with your supervisor.
- Provide supervision and training opportunities to Life Skills Instructors working with your caseload at Arete.
- Work independently, complete paperwork on time, solve problems when they come up, ask for clarity and present solutions.
- Support Arete Program Development: transportation, passion projects, daily activities, etc.
- Develop training strategies for Life Skills Instructors working with Arete Participants.
- Complete performance reviews for yourself and for peers when requested.
- Effectively communicate your schedule and maintain an up to date calendar.
- Use BambooHR to request time off and discuss with your supervisor for approval.
- Seek opportunities to add to your position, create functional systems, and ways to increase efficiency.

Expectations
- 40 hours per week.
- On-call rotation responsibilities.
- Follow through on the completion of projects, work, assignments, calls, and meetings.
- Meet with your supervisor weekly.
- Additional responsibilities may be assigned due to need, new responsibilities may be assessed by either the Program Director or the Director of Operations to ensure there is capacity for the new responsibilities within the role.
Qualifications:
- Master's Level Human Services training, such as MSW, LICSW, etc.
- 3 years experience working in a comparable work environment (residential treatment program, teaching, social services, social work and congregate living environment, etc.).
- Experience managing teams, schedules and logistics.
- Integrity, trustworthiness and professionalism. A strong ability to complete tasks independently and in a timely manner.
- Can quickly develop a complete understanding of how to use Google Suite Applications, Electronic Medical Record, and other Arete software programs.
- Must be able to pass extensive background investigation (National and Vermont Database checks).
Compensation:
- $55,000-75,000 DOE
- 200 Hours (5 Weeks) of discretionary paid time-off annually.
