Summary
Assists the Home Care Services office with the management of day to day operations including : facilitating communication within and between departments and with outlying facilities, electronic medical record workflow tasks, coordination of medical record requests, and managing phone coverage.
Position Reports To : Home Care Services Supervisor
Essential Functions
Answers the telephone in a timely manner and directs the calls to the appropriate personnel.Communicates with physicians and clinical staff.Workflow tasks; responds to assigned tasks in a timely manner for accurate maintenance of the patient episode of care and medical record. Sends required information to the appropriate facilities within the proper time frame.Ensures that appropriate summaries are sent to facilities and providers within regulatory time frames.Index documents in Worldview : reviewing orders to ensure signature and date by appropriate provider.Review and attach encounter notes to EMR and communicate any new orders or communication from providers to appropriate clinical staff.General office and paperwork tasks may include but are not limited to phone calls, sending and responding to emails, sending mail, faxing, printing, scanning, copying, data entry and tracking, updating patient demographic profile and adding attachments to the electronic medical record.Orders needed office supplies for all staff.Assists patients over the phone and in person.Collaborates effectively with a variety of people across departments.Exhibits excellent customer service skills.Maintain the confidentiality of patient information.Other duties as assigned.Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times.
Qualifications
Required :
High school graduate or GED.Must have 5 years of Health care related experience.Efficiency in software programs such as Microsoft Word, Excel, Outlook.Desired :
Medical terminology course.Prior computer experience.Previous experience in a medical or related type of facility or office.Physical / Sensory Demands

Many of the essential job functions of this position are done while seated at a desk. This may be in an open hallway or in a more confined space.
