Summary:Position Reports To: Practice Manager
- Manage, develop, implement and evaluate initial patient plan of care following clinical pathways in collaboration with the Urology Physician and the health care team, to determine the effective utilization of resources and outcome achievement.
- Facilitate and/or conduct patient, family, and team conferences to develop and monitor a plan of action as necessary. This includes patient education on the desease, treatment options, procedures and medication regimens and side effects.
- Ongoing assessment of patients for unmet social needs with appropriate referral and communication to social services, community resources, etc.
- Determine need for further diagnostic work-up; communicate and coordinate necessary tests or procedures to the patient and the healthcare team.
- Obtain, review and scan outside patient records for new newly referred urology patients.
- Coordinate simple and complex patient care issues in conjunction with other parts of the healthcare system, working interdepartmentally.
- Determine timeliness/time appropriateness of appointments.
- Ensures effective and appropriate utilization of resources and efficient patient througput along continuum of care.
- Document interim progress notes reflecting coordination of care/services. Intervene when the clinical pathway is not being followed, either due to patient constraints or ineffective follow-through.
- Provide traige services and symptom management to patients as needed, in-person, telemedicine or over the phone. Communicate syptomaology to appropriate team members.
- Required to maintain active Basic Life Support (CPR) certification.
- Other duties as assigned.
- Current licensure in the state of Washington (RCW 18.88) or licensure through Multistate Nurse Licensure Compact (SSB 5499).
- Current BLS Health Provider.
- Competent in computer software use and documentation in EMR.
- Three (3) years of clinical experience.
- Ability to work independently and perform role with minimal guidance.
- Urology experience.

General supervision is provided by the Practice Manager with additional instructions, guidance, and direction by the physicians or providers with whom the RN is working.
