Job Summary:
The Care Coordinator is responsible for supporting patients throughout their healthcare journey by organizing and facilitating care plans, coordinating services, and ensuring seamless communication among healthcare providers, patients, and families. This role ensures that patients receive optimal care, have their needs met in a timely manner, and are supported throughout their treatment and recovery.
Key Responsibilities:
Care Coordination:
- Develop, implement, and monitor individual care plans in collaboration with patients, families, and healthcare providers.
- Coordinate services across multiple care providers to ensure continuity and effectiveness of treatment.
- Communicate regularly with patients and their families to provide updates, answer questions, and address concerns.
- Ensure patients are educated on their condition, treatment options, and follow-up care needs.
Scheduling and Referrals:
- Schedule appointments, tests, and follow-up visits for patients.
- Ensure timely referrals to specialists and other healthcare services as required by the care plan.
- Coordinate transportation and other support services to ensure patients attend appointments.
Documentation and Record Keeping:
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations and privacy laws (e.g., HIPAA).
- Document patient interactions, progress, and any changes in care plans in the medical record.
- Track patient outcomes, report on progress, and adjust care plans as needed.
Advocacy:
- Serve as the primary point of contact for patients, advocating on their behalf to ensure they receive appropriate services.
- Assist patients in understanding their healthcare benefits, including insurance coverage, and facilitate access to needed resources.
Collaboration:
- Work closely with doctors, nurses, social workers, and other healthcare professionals to ensure a comprehensive approach to patient care.
- Participate in multidisciplinary team meetings to review patient progress and make necessary adjustments to care plans.
Follow-Up and Monitoring:
- Monitor patients' progress, ensuring they adhere to treatment plans and appointments.
- Provide reminders and motivational support to encourage patient engagement in their care.
Crisis Management:
- Respond to patient emergencies or concerns by providing immediate assistance and escalating to appropriate healthcare providers when necessary.
- Develop contingency plans for patients at high risk for complications.
Qualifications:
Education: Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field. (For nursing roles, a valid nursing license is typically required.)
Experience: Previous experience in a healthcare setting, particularly in care coordination, case management, or patient support roles.
Skills:
- Strong communication and interpersonal skills for interaction with patients, families, and healthcare teams.
- Organizational and time-management skills, with the ability to manage multiple tasks simultaneously.
- Proficiency in healthcare software systems and documentation practices.
- Knowledge of medical terminology and healthcare delivery systems.

Certifications (Optional but Preferred):
